Please remember that each athlete must have a current sports physical on file in order to participate in any activities. All new physicals must be on the new forms. Physical binders are located in Mrs. Celsor's office. Please remember that a copy must be filed in the front office, and the head coach must also keep a copy. The MS students who only participate in MS sports will now need two forms to be completed. Students who are participating in HS sports will need the one form listed. It is imperative that our student athletes receive their New Physicals on these new forms.
